Licensing Policy Notice
This Licensing Policy applies to all proprietary digital products offered by Perioperative Telehealth Evaluations™, LLC, including but not limited to the PTE™ Preoperative Prompt Toolkit series, AI-guided workflow tools, perioperative documentation resources, and future releases.
Individual Provider License
This product is licensed for use by a single licensed healthcare provider.
Permitted Use

Use is limited to the licensee’s own clinical practice.
Prohibited Uses

No redistribution, sharing, or transfer

No resale or sublicensing

No creation of derivative works (including modified prompts, templates, or workflows) for redistribution, resale, publication, AI training, institutional deployment, or use outside the licensee’s own clinical documentation

No use for training artificial intelligence models

No sharing with non-licensed users

No embedding, uploading, or integrating prompts into shared EHR templates (e.g., shared Epic SmartPhrases), institutional AI tools, shared drives, or automated documentation systems without a Clinic or Enterprise License

Violation of these terms constitutes unauthorized use.

Licenses are non-transferable and non-assignable.

Unauthorized use may result in license termination and legal enforcement.
Clinic / Group License
This product is licensed for internal use by a named clinic, group practice, or healthcare entity.
Permitted Use

Internal use by licensed clinicians within the purchasing entity

Use may be seat-based or site-based depending on purchase

No resale or sublicensing
