HOW IT WORKS

Perioperative Telehealth Evaluations™ represents one clinical implementation of the PTE™ Perioperative Optimization Framework.
The framework provides a standardized, evidence-aligned structure for perioperative readiness, risk stratification, and optimization—deployable across telehealth evaluations, facility workflows, and digital care platforms.

Through secure telehealth, PTE applies this framework to support individualized preoperative evaluations designed to streamline readiness, reduce avoidable delays, and support safe surgical care—while preserving clinician judgment and institutional protocols.

Step 1: Entry Into the PTE™ Perioperative Optimization Framework

All engagements begin within the PTE™ Perioperative Optimization Framework, a structured, evidence-aligned system designed to support perioperative readiness, risk stratification, and optimization across care settings.

Patients may self-start, or surgeons, anesthesiologists, or facilities may refer patients directly.

Telehealth evaluations represent one clinical implementation pathway of the framework.

PTE supports—not replaces—the surgeon and anesthesia team.

Step 2: Structured Data Intake & Information Collection

Consistent with the PTE™ Framework, standardized data intake ensures complete, audit-ready perioperative review while preserving clinician judgment.

Patients complete structured intake forms and upload available medical records, including:

Required (minimum dataset):

  • Planned procedure, surgeon, facility, and anticipated date

  • Medical history, medications, and allergies

  • Functional capacity and recent vitals

If applicable / available:

  • Recent labs (e.g., CBC, BMP, A1c)

  • Cardiac or pulmonary studies (EKG, echo, stress test, imaging)

  • Specialist notes or hospital records

If additional information is required, PTE coordinates secure record requests to support comprehensive framework application.

Step 3: Framework-Guided Pre-Visit Clinical Review

Prior to any patient encounter, the PTE™ Framework is applied to perform a structured clinical review designed to:

  • Identify perioperative risk factors

  • Flag missing, time-sensitive, or high-impact data

  • Determine whether further clarification or testing may be appropriate

This step ensures evaluations are focused, efficient, and clinically meaningful.

Step 4: Telehealth as a Clinical Implementation Pathway

As one implementation of the PTE™ Framework, patients may participate in a secure telehealth evaluation with a licensed Nurse Practitioner.

During this clinician-directed visit, the framework is applied to:

  • Review medical history and perioperative risk

  • Provide evidence-aligned optimization guidance

  • Address medication considerations and perioperative planning

  • Identify red flags requiring further coordination

Clinical judgment remains fully with the licensed provider.

Step 5: Standardized Framework Outputs & Care Coordination

Using standardized framework outputs, PTE prepares a structured perioperative summary that may include:

  • Risk stratification and readiness assessment

  • Optimization recommendations

  • Follow-up considerations, when appropriate

Reports are typically completed within 24–48 business hours after required records are received and are securely shared with the surgeon, anesthesiology team, or facility according to institutional preferences.

Step 6: Optional Educational & Optimization Resources

When appropriate, patients and clinicians may access optional educational resources and OTC perioperative tools through the PTE Marketplace.

  • Use of marketplace resources is never required

  • Educational tools do not replace individualized clinical decision-making

  • Access to resources does not constitute medical advice or treatment

Step 7: Follow-Up & Ongoing Framework Application (When Needed)

If additional clarification, optimization, or coordination is required, follow-up pathways may be initiated to support timely readiness and communication across the care team.

Follow-up remains individualized and framework-guided.
